The sophistication required to make this game harder, fairer, and better is no simple task. If you have preconceived ideas based on the many poor-quality Doom mods that exist, you should shed them before playing this mod. This mod removes many of the unfair elements and then makes the game more sophisticated, which, in turn, makes it much more difficult. The problem with classic Duke Nukem is that it was unfair in terms of enemy interaction, placement, environmental hazards, and unfair weapon ranges. The RPG Ultra has so much splash damage that it is difficult to use safely. Perhaps the most unfair addition is how many of your own weapons can damage you. Plus, the rebalanced skill levels help progression feel more natural to the difficulty of the game.

The extra life system helps to offset how difficult the game now is. You can also carry less ammunition for some weapons, and weapons like the RPG now have a physics-based velocity and a drop velocity. The mod adds a flamethrower, plasma gun, and the old alpha shotgun, along with adding alternate fire modes.

There are legacy weapons and enemies existing beside new enemies and new weapons.
